1 EXTERNAL POSTING (November 2015) The Canadian Mental Health Association, Ottawa Branch (CMHA) is a private, nonprofit charitable organization dedicated to promoting good mental health and improving the quality of life for individuals experiencing serious mental illness. The Community Support Services of CMHA provides outreach, long-term intensive support and housing to individuals with serious mental illness and substance abuse disorders who are homeless or at imminent risk of becoming homeless. CMHA Community Support Services are available to active clients of the agency 7 days a week. CMHA currently has one vacancy for the following position: EMPLOYMENT CASE MANAGER (Permanent) Type of Position: Flexible hours required Length of Term: Permanent Full-time (Please note at this time the Branch has only received a 3 year funding commitment) Hours of Work: Full Time, 35 hours per week, Monday to Friday 8:30-4:30 Pay Rate: $33.11 per hour to $36.44 per hour as per the Collective Agreement, CUPE Local Language: One position Bilingual (French/English) essential. *Please note that second language skills will be assessed by an outside agency. Please note as of January 2016, CMHA, Ottawa Branch will be participating in the Healthcare of Ontario Pension Plan (HOOPP). ESSENTIAL MINIMUM QUALIFICATIONS (Please see the attached position description for greater detail on position responsibilities and qualifications): Education: A University degree in Social Work (BSW) or equivalent in human sciences such as a B.A. in Psychology, Criminology, Occupational Therapy. Successful candidates will be required to have completed the on-line Certificate in IPS (Individual Placement and Support) training in order to successfully complete their probationary or trial period in this position. The employer will organize and pay for this certification training. Successful candidates will be required to attend on site job training and shadowing of
2 IPS Employment Case Managers in Toronto. The employer will pay for all costs associated with this training. Experience and Knowledge: A minimum of three years recent relevant clinical work, in the judgment of the employer, with vulnerable individuals, specifically people who have serious mental illness as well as individuals who are homeless, who have substance use disorders, and/or who have been in conflict with the law. 3 EMPLOYMENT CASE MANAGER POSITION DESCRIPTION Canadian Mental Health Association OTTAWA BRANCH Title: Employment Case Manager Program: Community Support Services Date Approved: September 2015 Reports to: Program Manager Approved by: Executive Director Date Revised: The Canadian Mental Health Association Ottawa Branch aspires to provide clientdirected services and programs with and for people with mental health problems and to enhance, promote and maintain the mental health of individuals and communities through education and awareness. POSITION SUMMARY: Under the direction of the Program Manager, the Employment Case Manager (ECM) works to create opportunities for individuals living with mental health disabilities to prepare for, obtain, and retain competitive employment, thereby increasing their economic participation and independence. Program clients will be assigned to an ECM as part of the Strengths Assessment Team Supervision process. The ECM will then undertake the development of a return to work action plan (RTWAP) for each participant. Based on client assessed interests and skills, the ECM will locate and develop job opportunities through assertive outreach to potential employers. The ECM will provide employment counselling, preparation and ongoing support to obtain and maintain the employment of their clients. The ECM will provide employment support services as part of the overall integrated treatment plan of the client, working in partnership with the Intensive Case Manager (ICM) and any other support services working with the client. PRINCIPAL RESPONSIBILITIES AND DUTIES: 1. Manage a caseload of individuals with mental health issues seeking competitive employment in a number of employment sectors. 1.1 Work with individuals in finding and maintaining competitive employment 1.2 Support and assists clients with developing and implementing an individualized Return to Work Action Plan 1.3 Provide linkages to community employment resources
4 1.4 Conduct employment readiness workshops with clients 1.5 Keep supervisor informed of any operational concerns 1.6 Work with the staff and clients to ensure successful job placements, including identification of accommodation requirements 2. Provide Job Development services 2.1 Identify and recruit potential employers through assertive outreach to the community and by conducting workplace environmental scans 2.2 Develop and implement strategies to increase employment opportunities and to reduce the stigma associated with Mental Illness in an employment context 2.3 Maintain positive employer relationships 2.4 Consult with employers to secure work placements, and to determine skill sets and educational requirements for jobs 2.5 Negotiate placement subsidy agreements with employers 2.6 Maintain communication with participating employers, monitor the success of clients and intervene with employers and clients accordingly 2.7 Network with other employment programs and community partners to expand resources 2.8 Prepare and distribute program marketing material and assist in other marketing activities such as presentations, networking and educational activities 2.9 Network with other employment programs and community partners to expand resources 2.10 Maintain employer/client documentation and statistical data 2.11 Meet required targets as identified by the funder for competitive job placements 3. Provide clinical intervention to support client employment needs 3.1 Complete all CMHA Employment Intake documentation in order to determine a client's personal and educational employment qualifications (strengths) and any barriers. Discuss soft and hard skills as well as short and long term goals. 3.2 Develop the RTWAP based on the Strengths Assessment of the client and other assessments as appropriate and requested by the client -- to identify resources and support which may include family members, other natural supports, and/or service providers. 3.3 Develop support, strategies and plans that reflect individual needs; record steps in an action plan that include a strategy on seeking and securing employment. Depending on individual needs, action steps can include referral to supported transitional employment, work experience, or further vocational/educational training.
5 3.4 Assist clients in adhering to their RTW Action Plan 3.5 Work with clients in meeting job performance requirements and in establishing positive workplace relationships. 3.6 Use coaching techniques such as prompting, modeling, reinforcements and teach self-monitoring techniques 3.7 Develop written list of work sequence instructions and the break down tasks 3.8 Train and support clients at the work site if necessary and in partnership / collaboration with available peer job coaches. 3.9 Review all active files at designated intervals to assess for ongoing involvement. In consultation with the direct supervisor and other appropriate CMHA team members, develop recommendations for ongoing employment case management services Provide immediate and appropriate interventions in situations where clients are at extreme risk (e.g. suicide intervention) and communicate these and any other urgent concerns to the client s ICM in a timely and clear manner. 4. Complete and maintain all file management and reporting functions in a comprehensive and timely manner 4.1 Complete all required recorded client contact appropriately 4.2 Complete monthly, annual, and other required documentation 4.3 Complete all appropriate assessments and treatment plans 4.4 Complete relevant documentation related to client awareness and understanding of rights relating to services and confidentiality 5. Conduct job related activities in accordance with the objectives of the program. 5.1 Provide strengths based and recovery focused support to clients in achieving goals, particularly in areas related to gaining competitive employment. Participate in the evaluation of the program goals and objectives making recommendations on changes to current program activities for improved service delivery. 6. Assist other agency staff where skills are applicable 6.1 Provide additional support and/or back-up to colleagues as required 6.2 Co-facilitate skill training, educational, or support groups for clients with CMHA Team members as appropriate 6.3 Participate in the development and delivery of educational presentations dealing with employment related services. 6.4 Participate in special events and activities for agency public education campaigns such as Mental Health Week and Mental Illness Awareness Week.

Maintain up-to-date knowledge of the Mental Health Act and related legislation, the structure and personnel of local social agencies, government departments, institutions and other groups concerned with the client population, and trends and development in the community that could affect the provision of mental health and employment related services 8. Deliver service in a way that maintains personal safety and the client s physical, social, cultural, and emotional well being. 8.1 Develop the appropriate skills and knowledge through ongoing training. 8.2 Deliver that service in a way that complies with specific legislation, standards of practice, and the policies and procedures of the agency. 9. Participate and contribute toward the effective working of the team and overall operation of the agency. 9.1 Participate in regular staff supervision, client reviews, team meetings, information and referral assignments, planning activities and/or other activities or special assignments as directed by the Program Manager/Executive Director 9.2 As a team member, accept joint responsibility for decisions reached by these processes 10. Provide support and/or supervision to agency volunteers and students assigned to the program area 10.1 Provide regular supervision, advice and support to volunteers and student placements in program and service area 10.2 Evaluate performance of volunteers and students in the program area 11. Avail self of opportunities for professional development 11.1 Bring forward training needs and take responsibility for seeking out relevant training opportunities 11.2 Participate in workshops and seminars as required 11.3 Complete all mandatory training (e.g. AODA, Worker Health & Safety Awareness 12.
