1 Five Counties Children's Centre is dedicated to supporting children and youth with physical, developmental and communication needs and their families. In partnership with families and communities we strive to enrich their independence and quality of life. Our Rehabilitation Team includes health care professionals and support staff. We work closely with the child / youth and family to provide assessment and treatment to meet their individualized goals of the families. We offer Outpatient and Community Based. Outpatient Population Served Settings Days/Hours of Service Frequency of Payer Fees Referral Children aged 0 6 are eligible for this program Outpatient services are provided at the Centre either with one to one or group treatment. Children aged 6 (Grade 1) 19 (graduation) are eligible for this program. The Centre is open Mondays Fridays from 8:30 a.m. 4:30 p.m. Some groups or other services may be offered in the evenings and weekends based on the individual needs of the child/ youth and the availability of the employees. Frequency of services is determined following the assessment, in consultation with parents, and documented in the care plan. These services are funded through the Ministry of Children and Youth, the Five Counties Children s Centre Foundation and the Ministry of Health and Long Term Care (Hospital for Sick Children). The Centre s are funded by the government, and the foundation, at no cost to the family. There may be fees for some educational workshops and recreation programs. While government programs provide some financial support for equipment for the child, there may additional costs for the family for which the Centre would provide support in seeking assistance. Families and other professionals (with parent consent) can refer directly for all service, with the exception of the Medical Director for which physician referral is required.

Community based services are also provided at school until children enter grade 1. Children aged 6 (Grade 1) 19 (graduation) are eligible for this program. School Health Support are provided in the schools. Therapeutic Recreation supports the inclusion of youth in recreation settings. Resource Teacher Program supports children to age 12 in licensed child care. Days/Hours of Service Community based services are provided Mondays Fridays from 8:30 a.m. 4:30 p.m. Some groups or other services may be offered in the evenings and weekends based on the individual needs of the child/ youth and the availability of the employees. School Health Support are provided within the hours of operation of the schools that we serve. These services are provided during the hours required by community recreation programs. Frequency of Frequency of services is determined following the assessment, in consultation with parents and caregivers, and documented in the care plan. The service model including the frequency of services is determined by the Central East and / or South East Community Care Access Centres.
