1 2016 Rollins Benefits Update Frequently Asked Questions Open Enrollment November 2 November 19 This list is designed to provide you with answers to some of the most commonly asked questions. If you have questions about details of the plan options or enrollment, please contact the Call Center, where you can speak with a licensed benefit counselor who can help you learn more about your benefits. Benefit counselors will be available by phone at from 7 am to 10 pm Eastern Time Monday through Friday, and 10 am to 2 pm Eastern Time on Saturdays. Saturday hours are available through December 5. 2 What happens if I don t enroll? If you don t enroll before Open Enrollment ends on November 19, 2015, you won t have coverage for All employees need to enroll regardless of whether they plan to elect coverage. Moving to Mercer s Enrollment Platform What is the new online platform? Mercer s online benefits platform is an enrollment site where you will elect your Rollins benefits during Open Enrollment, November 2 19, On the site, you ll choose from an expanded selection of benefits including medical, dental, vision, disability and life insurance, supplemental medical, and much more. You can easily shop for plans that make sense for you and your covered family members. You ll also have access to Mercer s call center with experienced licensed benefit counselors to answer all your benefit questions. Why are we moving to Mercer? Rollins has a self-insured medical plan. That means, we pay our carrier to process claims from providers. We also pay for access to heavily negotiated discounts, so our employees pay less when they go to the doctor. Rollins shares in the cost to process the claims with our employees through premiums roughly 60% is paid by Rollins and approximately 40% is paid by employees. In the past five years, premiums have increased an average of 10%, which has resulted in higher costs for employees and the Company. As our business continues to grow and the cost of providing health benefits continues to rise, moving to Mercer gives us a simpler, more efficient way to offer a comprehensive and competitive benefits program. In addition, employees will have some medical plan options that cost less than our current plans. We shop for everything in our daily lives. Why should shopping for benefits be any different? We want to make it easier for you to select the right coverage at the right price, manage your expenses, and take charge of your well-being. Through the enrollments site, we re able to offer you a wider range of benefit plans and cost options, with greater flexibility to create a customized benefits program that fits your needs and budget. In addition, you ll have a more convenient way to learn about your benefits and enroll. Mercer s website offers an easy, guided shopping experience and built-in tools to help you make your benefit decisions. You ll also have access to a team of experienced benefit counselors, available by phone. Benefit counselors are available by phone at from 7 am to 10 pm Eastern Time Monday through Friday, and 10 am to 2 pm Eastern Time on Saturdays. Saturday hours are available through December 5. 2
3 Benefit Changes Employees Can Expect How are my benefits changing for 2016? Rollins employees will have access to a new and expanded range of benefit options for 2016, which means changes for everyone. When you enroll for benefits, you will have: Five new medical plan options ($800 Deductible, $1,500 Deductible, $2,850 Deductible, $4,500 Deductible, and $6,550 Deductible); three Health Savings Account (or HSA)-eligible plans and two traditional PPO plans: o These plans offer a range of deductibles and premiums per pay period. o Anthem will continue to administer the medical plan for most of our employees. For others, about 20% in select cities, we have chosen UnitedHealthcare (or UHC) to administer the medical plan. UHC offers stronger networks and better discounts for in-network care in those locations. Three new dental plan options (Basic Plus, Enhanced, and a DHMO) through Cigna plus two new vision plan options (Standard and Enhanced) through EyeMed. Health Care and Dependent Care Flexible Spending Accounts (FSAs) administered by Discovery Benefits, our new administrator. We want to encourage employees to spend what s left in their FSAs by December 31, 2015 since we will be moving to Discovery Benefits. Basic Life and Accidental Death & Dismemberment (AD&D) insurance and new Voluntary Term Life (or VTL) options for employees and dependents, through Liberty Mutual, as well as Short- and Long- Term Disability. Employees will have a special, one-time opportunity to enroll in disability coverage without having to complete a medical questionnaire. Employees will not be denied even if they have been turned down before. New voluntary benefits, including Critical Illness, Accident, Hospital Indemnity, Auto & Home, Legal, Identity Theft Protection, Pet Insurance, and Perkspot discount shopping mall, which will replace Rollins Advantage. What other changes can I expect for 2016? In addition to changes to our benefits, we are also making a few updates to our eligibility and enrollment rules: We will continue to require proof of eligibility when enrolling new dependents for coverage. However, Rollins will grandfather all existing spouses and children currently listed as dependents. We will require employees who enroll a new or currently enrolled domestic partner to provide a domestic partner state registration issued by the state in which they live. If an employee is unable to provide the certification, he/she will not be able to enroll a same- or opposite-sex domestic partner. Rollins will continue to recognize all legal marriages. We will also no longer allow benefit changes in the first quarter of the new plan year. Changes will only be allowed for a qualified status change, like a birth of a child, marriage, etc. No exceptions. New hires in 2016 will have 30 days to enroll for benefits instead of 60 days. 3
4 What s different about the new medical plan options and prescription drug coverage? There are several important differences about the new medical plan options and prescription drug coverage offered to you for 2016: One of the biggest changes you ll notice is how the medical plans work. There are no more copays. Instead of paying a copay for doctor s office visits and hospital stays, you ll pay coinsurance, which is a percentage of the cost of your care. Coinsurance applies after you ve paid your plan s annual deductible. Keep in mind that all preventive care, like annual physicals and immunizations, is still covered at 100% in-network at no cost to you. Depending on the plan you choose, you may be able to contribute tax-free money to a Health Savings Account. With an HSA, you own the account and keep any unused money year after year. In addition, Rollins will put money into your HSA to help cover your out-of-pocket expenses. The medical plans do not have a separate prescription drug deductible. After you meet the medical plan deductible, you will pay coinsurance instead of copays for mail order prescriptions (90-day supply).you will continue to pay coinsurance for retail prescription drugs (up to a 30-day supply) after you meet the medical plan deductible. Plus, the cost of prescription drugs will count toward the medical plan deductible in the HSA-eligible plans. In addition, if you or your provider requests a preferred or non-preferred brand drug when a generic alternative is available, you will be responsible for the difference in cost between the brand and generic drug. Will I have to change doctors? Most of our employees will have Anthem as their carrier. For 2016, we are adding a second medical plan administrator UnitedHealthcare (UHC) to service about 20% of our population. In some cities, UHC simply offers stronger networks and better discounts for in-network care. Anthem will remain our partner in all other locations. Employees who are located in an area where UHC will be their new administrator, received targeted communications about this change. While some employees will need to find new doctors, 97% of doctors currently used by employees participate in the UHC network. Rollins Benefits.com includes a list of cities, in which UHC will be the new administrator, and how to search for doctors on the UHC site. 4
5 What s the difference between the new Health Savings Account (HSA) and the Flexible Spending Account (FSA) for health care that we have today? HSAs work like FSAs, but offer higher contribution limits than the FSA our employees are taking advantage of today. Here s a comparison of the two accounts: HSA vs. Health Care FSA Available to Employees: Annual contributions: Use it or lose it? HSA Enrolled in the $2,850 Deductible, $4,500 Deductible or $6,550 Deductible Plan Rollins contributes $200 to your account You and Rollins may contribute a total of: o Employee only $3,350 o Family $6,750 If you are age 55 or older, you may contribute an additional $1,000 No, the money in your account is always yours to keep Health Care FSA Enrolled in the $800 Deductible or $1,500 Deductible Plan Not enrolled in a Rollins Medical Plan You can contribute up to $2,550 Yes, money cannot be carried over to the next year If I m enrolled in EE Only Medical coverage in the $2,850 plan, and my children are covered under my spouse s employer s medical plan, can I use my HSA to pay for my kids medical bills? Yes, you can use your HSA to cover any for out-of-pocket expenses (i.e., Deductibles, Office visits, Prescription drugs, Hospital stays and lab work, Speech/occupational/physical therapy, Dental and vision care, etc.) for anyone you can claim as a dependent on your taxes. Does my HSA carry over to the next plan year? You own the account. Unused money can be carried over each year and invested for the future. IMPORTANT NOTE: FSA IN 2015, HSA IN 2016 If you are participating in a Health Care FSA today, and plan to enroll in the $2,850, $4,500 or $6,550 Deductible Plan and open an HSA for 2016, you ll need to use all of the money in your FSA by December 31, 2015 in order to participate in the HSA beginning on January 1,
6 Is the amount that I defer for the entire plan year available immediately in my account? No, an HSA works like a bank account. You are able to use the money as you accumulate it in your account. How do these new benefits save me money? The amount you spend (or save) will be determined by the elections you make during enrollment and who you cover. Some employees will see significant savings. Here are a few examples. If you re enrolled in family coverage o In the 2,000 PPO Plan today, and you enroll in the $2,850 Deductible Plan for 2016, you will see an 8% decrease in your monthly premiums. o In the PPO Max dental plan today, and you enroll in Basic Plus for 2016, you will see a 21% decrease in your monthly premiums. o In the Enhanced vision plan today, and you enroll in the Enhanced plan for 2016, you will see a 38% decrease in your monthly premiums. Mercer s site can help you take control of your health spending by: o Clearly showing the deductible and out-of-pocket costs associated with each plan. o Helping you find the most cost-effective plan for your needs. o Offering tax-saving opportunities through health and dependent care accounts. o Giving you access to group discounts on additional benefits, like auto and home insurance, pet insurance, and more. Information and Decision Support Available to Employees Where will I find information about my new benefits? This year, you will learn about your benefits through Rollins Benefits.com, The site: Features an introductory video from John Wilson, our President and Chief Operating Officer, previewing the coming changes. Provides the information you need to understand your new benefit options this year and see your coverage costs. Features a video on the HSA, an interactive tool that helps compare the plans and more. It is accessible 24/7 from work or home and be the way you get to Mercer s enrollment site. It will also be available after Open Enrollment as a year-round resource. How will Mercer s enrollment site help me make decisions? As you enroll, you ll find key information displayed for each plan, including coverage details and what it will cost. You ll also find tools, educational videos, and other information to help you understand your benefit options better. 6
7 If you need help finding the right coverage, you can use the Mercer Marketplace decision-support tool. First you ll be asked a few questions about your medical insurance usage, payment preference (e.g., do you prefer to pay more out of pocket when you go to the doctor or more for coverage through premiums per pay period), and ability to afford an unexpected medical expense. Then, the website will show you one or more plans that may best match your situation. While the decision is yours, these suggestions may help you make an appropriate choice. Plus, you ll have access to the Mercer Marketplace Plan Shopping App, which allows you to compare two or more medical plans and model scenarios personalized for you based on your individual situation. What should I consider before I enroll in my 2016 benefits? We encourage you to give some thought to these questions before you enroll: How much health care and what type of care did you and your covered family members need in 2015? Will your needs be similar in 2016? Do you foresee changes? Do you have an upcoming surgery scheduled, will you need more or fewer prescriptions? How do you prefer to handle costs? For instance, would you rather pay more from your paycheck for a medical plan that covers more of your costs when you need care, or pay as little as possible from your paycheck even if that means bigger bills when you need care? How much life and disability insurance do you need to feel comfortable and well protected? Do you need supplemental medical coverage like Critical Illness, Accident, Hospital Indemnity? You ll learn more about these benefits on Rollins Benefits.com. How and When to Enroll When is Open Enrollment? This year s Open Enrollment will be held from November 2, 2016 through November 19, You must enroll to have coverage, since many of our current plans will not continue in This means that if you do not enroll by the deadline, you will not have Rollins benefits coverage in No exceptions. We ask that all Employees go through the enrollment site regardless of whether they plan to elect coverage so they can elect a beneficiary for the company-provided Basic Life and AD&D coverage. How do I enroll? Visit the RollinsBenefits.com website and link to the Mercer Marketplace enrollment website to enroll in your benefits for The site will guide you through the enrollment process step by step. You will be able to use the decision tool and answer five simple questions to help determine the coverage you may need. You will see the coverage details and costs for each plan before deciding what to put in your shopping cart. When you check out, you ll have a chance to review your selections and see your total cost on a per pay period basis. It s that simple! You ll also have access to a team of experienced benefit counselors, available if you need assistance understanding the different plan options or prefer to enroll by phone. 7
8 What happens if I don t enroll? If you don t enroll before Open Enrollment ends on November 19, 2015, you won t have coverage for We will also no longer allow benefit changes in the first quarter of the new plan year. Changes will only be allowed for a qualified status change, like a birth of a child, marriage, etc. No exceptions. Where do I enroll for benefits if I m newly hired? If you are hired: Before or on October 1, you will need to enroll in benefits for both 2015 and Please follow the directions you ve been provided for enrolling in 2015 benefits. If you have questions, please call the Benefits Line. Then, beginning on November 2, you will enroll on Mercer Marketplace for Between October 2 November 1, you will enroll for benefits during 2016 Open Enrollment with current employees on Mercer s enrollment site. November 2 December 31, you will enroll for on Mercer s enrollment site. We will provide additional information about the enrollment process to all new hires during this time period. Prescription Questions How do I know if my medication is formulary? Members can check if their medication in on the new formulary by accessing the OptumRx Public Portal: What happens if I have a Prior Authorization in place? If a member has a current prior authorization in place with Express Scripts it will transfer to OptumRx and remain active until the expiration date. The member will be required to receive a new prior authorization once the current prior authorization expires or if the prior authorization expires prior to 01/01/2016. What happens to my refills currently with ESI s home delivery? Will I have to get a new script? If a member s current mail order prescription with Express Scripts has one or more refills available as of 01/01/16, they will automatically be transferred to OptumRx. There are some circumstances where a script will not transfer such as narcotics or medications that do not have refills available. If a member s medication does not have any remaining refills or does not qualify for an automatic transfer, ask your doctor or prescriber to write a new 90-day prescription after 01/01/16. 8
9 Supplemental Plans: How do the new Aflac plans work? All three of the new Aflac plans provide flat payment amounts that are paid to you if you are either in an accident, hospitalized, or diagnosed with a specific critical illness (see RollinsBenefits.com for specific illnesses covered). Are there any limitations to eligibility in the Aflac plans? Only critical illness has limitations to eligibility. Aflac will not cover a specific critical illness if you have received treatment for that critical illness in the past 12 months. Do these plans replace my major medical coverage? All three of the Aflac plans are to supplement your major medical coverage, not to replace it. The Hospital Indemnity plan says HSA can it be used with FSA eligible plans? Yes, the Hospital Indemnity plan can be used as supplemental coverage with ALL plans. 9
